The Durham County Department of Public Health’s Vital Records office processes Durham County birth and death information (This office DOES NOT issue certificates of any kind) for COPIES OF CERTIFICATES REFER TO THE REGISTER OF DEEDS OFFICE.
In the Vital Records office, you can:
1- Add last names to the birth certificate by completing an affidavit.
For this, both parents must be present, and you must bring:
• Copy of birth certificate and
• Some accepted ID:
- Passport
- or Real ID
- or Military ID
2- Send information about home births to be processed. For this you must:

3- Deliver Notification of Death information and Certificate of Death to be processed
4- Get a form for disinterments/ reinterments, burial transit and removal of graves. For the forms, click here: LINK
REMEMBER: Durham County Public Health is a NON-ISSUANCE office to the public. You can ONLY get certified copies of Birth Certificates, Death Certificates, Amendments, Legitimations and Marriage Licenses at the DURHAM COUNTY REGISTER OF DEEDS OFFICE, regdeeds@dconc.gov

You can only get certified copies of Birth certificates, Death certificates and Marriage Licenses at The Register of Deeds. regdeeds@dconc.gov |
Please Note: Certified copies of birth and death certificates, amendments and legitimations can only be obtained at the Durham County Register of Deeds Office.
