Population Health at Durham County Department of Public Health for Website
The Association of State and Territorial Health Officials defines population health as the health outcomes of a group of individuals including the distribution of such outcomes within the group with three main components- health outcomes, health determinants and policies.
The Durham County Department of Public Health Population Health Division focuses on community engagement, data, quality improvement, workforce development, strategic planning, accreditation and policy development.
The division will use many strategies to improve health outcomes of Durham County residents, centering data and equity such as:
- Community Centered Approaches
- Community-driven health priorities (Community Health Assessment & listening sessions/Partnership for a Healthy Durham)
- Integrate residents’ stories to contextualize population health statistics
- Expanding User-Friendly Public Data
- Develop data dashboards that highlight existing health inequities and progress in addressing community-driven health priorities
- Develop infographics and reports to disseminate data on health inequities and priorities
- Policy Development
- Intentional focus to increase policy outcomes that maximize population health
- Engage community partners in conducting policy analyses to determine which polices will maximize population health
- Community Engagement:
- Engage the community in an authentic way that acknowledges their expertise
- Create a community advisory council
- Obtain National Public Health Department Accreditation
- Meet the requirements of the many population health and equity standards
